The prevalence of this disease worldwide is estimated to be 35% and in Asia it is estimated to be 30-50% in adolescents. Like most chronic liver diseases, most people with asymptomatic non-alcoholic fatty liver are often asymptomatic, and only after finding abnormal laboratory tests, or because of sonography for various reasons, such as gallstones, fatty liver disease, diagnosis given. So far no remedial treatment has been proposed for this disease and there is no proper and ideal treatment. Medicinal plants have always been considered as an alternative to industrial medicines due to ease of access, reduced side-effects and reasonable prices. So, one of the most effective herbal medicine to treat liver diseases is Shelliver.

Pharmacological effects of Shelliver (7 medicinal plants)

Liver Diseases

One of the reasons for liver damage in biliary obstruction, is to increase bile acidity and change the oxidant balance of the antioxidant which can lead to multiple complications by stimulating lipid peroxidation. In addition, oxidative stress usually increases in cirrhosis (Kume, 2004; Oyedemi, 2011). Also Cichorium intybus and Silybum marianum contain large amounts of anthocyanins vitamins A and C that can reduce inflammation and cholestatic complications.

In addition, by reducing the activity of oxidizing agents (MDA) malondialdehyde and increaseing the activity of antioxidant agents like (SOD) dismutase superoxide and (GHS) Glutathione hepatic, shelliver can protect the liver against carbon tetrachloride fibrosis.

Polyphenolic compounds that present in shelliver can protect liver cells against damages of liver toxins and free radicals (Sakihama, 2002). This therapeutic properties are achieved through the stability of the cell membrane and preventing the oxidation of membrane lipids by active.

Also it can decrease liver toxicity by modifying hepatic enzymes such as serum aminotransferases SGOT, SGPT, Alkaline phosphatase and bilirubin.

Skin problems

A beneficial liver enhancer (Shelliver) is useful for restoring skin health as complications of fatty liver. Shelliver compounds play a significant role in facilitating and strengthening the liver and treating its diseases and eliminating the skin problems that caused by liver toxins. Also by applying the antioxidant mechanism and the elimination of toxins and zeros as well as the neutralization it provides cell regeneration and rejuvenation for the liver and prevents skin problems (Luper, 1999). Also by lowering gallbladder in blood, Skin complications such as psoriasis, hives and allergies as well as eczema and psoriasis are eliminated. These valuable and beneficial effects depend are on the effective ingredients including alkaloids, Prothyridine, Silymarin, Traxanthin, vitamin A, B and C. (Vogel, 1975).

In liver disease, biliary obstruction usually causes, deposition of fat and bile secretions, secreted into the bloodstream and their symptoms are in the form of liver spots, skin jaundice, itchy skin and brown spots. In this condition, the liver cannot absorb free radicals from the blood, thus the skin problems increase. One of the ways to reduce these radicals in the body is taking antioxidant-rich medications that prevents the cells from being destroyed. One of these herbal medicine is Shelliver. Based on all plants used in Shelliver with antioxidants effects it can produce bile eliminate toxins from the liver, reduce the destruction of the walls of the liver cells and purify blood.

Thromboxane (TXA3), Lecturers (LTBS) and prostaglandin in the Shelliver through the mechanism of increasing the secretion of bile acids and the reduction of stool natural stress can lead to improved skin problems. Shelliver compounds stimulate collagen production and shorten wound shrinkage (McDaniel, 2008), angiogenesis, vascular dilatation, decreased inflammation as well as skin problems.

Silymarin significantly reduces the number of interleukin-producing cells 10, as well as its production in the inflamed skin of ultraviolet radiation. It is also able to inhibit macrophage activating Lipopolysaccharides.

Silymarin is also traditionally used to treat psoriasis. The effect of silymarin for improvement of psoriasis due to the elimination of unwanted metabolites of body cells especially the liver as well as inhibition of the cycle cAMP and inhibiting the synthesis of Leukotrienes. It increases cycle cAMP time in patients with psoriasis and the synthesis of Leukotrienes whose containment is likely to result in a recovery of the disease (Koch, 1985).

Shelliver is a combination of lactobenic-containing compounds Doxy lactosine 8, a potent inhibitor of cyclooxygenase type two (Hassan, 2008), as well as some of the existing flavonoids and anthocyanins stimulating the immune system and limiting infection and inflammation (Anis,2001).

Treatment of jaundice

Abnormal increase in bilirubin concentration in the blood, which is due to a poor functioning of the liver creates a yellow color in the skin and eyes. Shelliver can reduce bilirubin and the levels of Malondialdehyde the liver is reduced and the levels of albumin, protein and levels of antioxidant liver enzymes increase, resulting in improved disease.

Blood glucose control

Insulin is an antitryptic hormone in the case of insulin resistance it interferes with various pathways in steatosis and lipid accumulation in the liver. In normal situation fatty acids have been transferred to the liver through the blood stream, there by one of the important liver cells called mitochondria which are the source of cellular energy production, changes in phospholipid are removed from the liver. This process is regulated by the effects of hormones and especially insulin. The accumulation of fat in the liver cells occurs when the production of fat is increased and their secretion from the liver is disturbed. This phenomenon occurs when the amount of fat in the liver is increased or due to dysfunction in mitochondria, the process of production and secretion of phospholipids is reduced. Insulin resistance is one of the most important factor. People with diabetes or obesity has insulin resistance which this increases the amount of fatty acids in the liver. The accumulation of these substances in the liver can be destructive and can lead to the death of the liver (Schmitz-Peiffer, 2000). By weakening the factors that lead to the excretion of fat from the liver cells (oxidation and fat fuel) and overcoming the factors that increase fat intake to the liver (Insulin resistance) the accumulation of fat inside the liver cells is caused.

Effective components of each medicinal plants used in Shelliver can affect the liver chemical intermediates including diponectin (an important factor in liver susceptibility to insulin) and increase its activity. On the other hand it also can decrease the activity of another mediator called TNFα , by preventing the entry of fatty acids into the liver cells, increased fat metabolism inside the liver cells, preventing the accumulation of fat in the liver and thereby reducing insulin resistance (Khan, 2003). Also, phenolic compounds and flavonoids can reduce the levels of aspartate transaminase enzymes (AST) (Kalailingam, 2011) and Alanine Trans-Amina Z (ALT) (Aydin, 2001). Revitalize the liver cells against glutathione emptying or by increasing the antioxidant enzymes of glutathione reductase Glutathione peroxidase and catalase. on the other hand these antioxidant compounds including flavonoids quercetin inhibit the absorption of glucose in the intestine (Loew, 2002) this action is done specifically on the GLUT2 vector (Singh, 2012) and Acetyl coenzyme A from pirovic acid instead of entering the triglyceride synthesis step it enters the krebs cycle (citric acid cycle), by lowering the blood glucose level and storing glucose to the glycogen. It ultimately leads to blood sugar control.
